Care Team Referral A palliative care consult is recommended by the team physician or nurse practitioner. Patient/Family Acknowledgement The patient and/or family are informed and give verbal acknowledgement of the referral. Fax Referral Referral form and supporting documents are faxed to 312-598-1514 Consultation Within 48 Hours A Serenity Health Group NP will visit for an in-person consultation and care evaluation.
